Terms and Conditions of Enrolment
Please read these Terms carefully.
I agree that an educator must be notified of the arrival and departure of my child.
My child is my responsibility until handed over to a staff member on arrival and once I have made physical or verbal contact with my child.
I agree to sign my child in and out daily and understand that not doing so will put my eligibility to receive childcare subsidy at risk.
I understand that there is a notice period to cancel my child’s care (four weeks, in writing) and flexible days are not possible.
Fees are to be paid in advance, at all times. I understand that I must stay up to date with my entitlements from Centrelink and the service does not have responsibility for tracking this.
I understand that Keiki Early Learning groups by developmental stages of children and free flows where possible. This means that children will be in environments that are developmentally appropriate for them. Educator rosters are informed by prescribed ratios and the age and developmental readiness of each child. I understand, the ratios within the services are as follows 1:4 Babies(0-18 months) 1:5 Toddlers (18 months - 2.5 years) 1:10 PreKindy and Kindy (2.5 years to 5 years)
I agree that my child’s image will be used on the daily program, Xplor Posts and social media. (If you have previously notified the service in writing or via the enrolment form otherwise this setting will not change).
I understand that my child will participate in a range of play experiences, that include but are not limited to bouncy castles, woodwork, coloured hairspray, make-up, face paints, nail polish and fake tattoos. (unless I have expressed in writing or on the original enrolment that my child is not to participate in any of these).
I understand that a new Health Care Plan must be provided at re-enrolment. If a new form is not provided, I understand that my child may not be able to attend the service.
I agree that all changes to Medical Conditions require written communication from a Medical Practitioner and that Medical Action Plans and Health Care Plans must be updated at least every 12 months or at re-enrolment.
I agree that I will supply the service, on enrolment, re-enrolment and thereafter as and when requested by the service, with a then current immunisation certificate (in a form approved by the service, e.g an immunisation history statement from the Australian Immunisation Register) showing my child's current immunisation status. I agree that provision of this information is a condition to my child continuing to attend the service, as is ensuring that my child's immunisation status is up to date at all times according to the Western Australian immunisation schedule. If at any time my child's immunisation status is not up to date according to that schedule and I cannot produce evidence of an approved catch up plan from a recognised immunisation provider or evidence that my child is otherwise exempt from immunisations under the Public Health Act 2016 (WA), my child's enrolment will be cancelled and I will be liable for any cancellation fees.
I agree to follow the conditions and responsibilities as set out on the Family Handbook and the Policies and Procedures at the Keiki Early Learning, which I have been provided access to via this re-enrolment and via email at enrolment.
